FIRST VISIT CHECKLIST
Provide the following prior to your first visit and your dog will be ready to play!

  • Dog is at least 4 months old, neutered or spayed if over 8 months old, in
    good health, rid of fleas and current on a flea preventative

  • Dog Personality Profiles for each dog  Excel | PDF

  • Owner Agreement and Release Form  Excel | PDF

  • Proof of Rabies, Distemper and Bordatella Vaccinations
    (We will contact your vet to obtain these records)
